I give permission for my child_____________________________ to participate in the following activities for the 2004-2005
school year and summer school/extended year 2005, if available. I have reviewed the information describing these activities
and checked those my child may participate in. I know that I may withdraw this permission by contacting the principal.

SWIMMING: Water adjustments, swimming experiences, and instruction are provided periodically (approximately 5-10 times yearly)
to students who have permission. Flotation devices are provided and used when necessary.
MEDIA PICTURES: Photographers from local newspapers or television stations may come to some newsworthy activities in
the community or at our school. By giving this permission your child's picture may be taken by such individuals. They may
then be used in a news story.
SCHOOL PICTURES: Each year in the fall a professional photographer comes to our building and takes individual school
pictures of all students with permission. These pictures are available for purchase and used for class pictures.
FIELD TRIP/TRANSPORTATION: This permission allows your child to be transported by school approved transportation for
field trips. You will continue to be notified by your child's teacher about the dates and plans for field trips.
HEADSTART: If your child participates in a classroom that shares space or activities with Headstart, it may be important
for AEA 7 staff to discuss particulars of your child's program with Headstart staff.
